What are the most common types of ailments/conditions that you treat?

I treat neck pain, back pain, shoulder pain, hip pain, headaches, postpartum issues, and chronic pain, as well as other general orthopedic issues.

Do I need a doctor’s referral or prescription?

You do not need a referral. However, some insurance companies require a referral and/or a pre-authorization before a physical therapy evaluation is completed. It is your responsibility to check with your insurance company on the requirements related to physical therapy treatment.

What do I need to wear to my session?

Wear comfortable, stretchy clothes that give you the ability to bend and move easily. Form-fitting, athletic clothing is best because I can identify postural and alignment positions and ensure proper form more easily. It is also important that you are comfortable, so always wear what what makes you feel the most comfortable. For pilates, you will work barefoot or with special grippy socks.

Do you accept insurance?

Restore Manual Therapy and Movement Training is a fee-for-service clinic and payment is required at the time service is rendered. If you are being seen for skilled Physical Therapy services, I will provide you with an itemized bill and coding information upon request. You can then file a claim with your insurance company and possibly be reimbursed under your out-of-network benefits.

Would it be a smart investment to see an out-of-network provider for Physical Therapy?

As a physical therapy patient, you are able to decide what therapist you use for your therapy. When making a decision about which physical therapist is best for you, there are several things you should consider:

  1. You should consider the amount of one-on-one treatment time you receive from a licensed Physical Therapist, not support staff.

  2. How accessible is your physical therapist? Will you be able to contact him/her for questions easily?

  3. How long will your appointments take?

  4. How much is the physical therapy clinic charging you or your insurance company? What is your yearly deductible or copay?

Taking all of these factors into consideration, Restore Manual Therapy and Movement Training, offers a higher VALUE for your money because you receive more time, care, and one-on-one attention from an experienced Physical Therapist with 24+ years in the profession.

What is your privacy policy?

Shannon takes her privacy policy seriously and is bound by HIPAA laws just like all medical employees, and she prescribes to the code of ethical conduct outlined by the American Physical Therapy Association.

Restore Physical Therapy’s privacy policy states: “I understand that health information about you and your health care is personal. I am committed to protecting health information about you. I create a record of the care and services you receive from me. I need this record to provide you with quality care and to comply with certain legal requirements. This notice applies to all of the records of your care generated by this practice. This notice will tell you about the ways in which I may use and disclose health information about you. I also describe your rights to the health information I keep about you, and describe certain obligations I have regarding the use and disclosure of your health information.

What is your cancellation policy?

Shannon’s schedule stays very busy with patients, but she also understands cancellations are inevitable at certain times. Cancellations for appointments made before 24 hours of the scheduled time may be done at no charge by contacting Shannon directly at (662) 418-3108. Late cancellations or no shows for appointments of less than 24 hours will be charged for the full amount of the scheduled appointment. If there is an emergent reason for a late cancellation (medical emergency, illness, death in the family), please contact Shannon at (662) 418-3108 before the start time of your appointment. Please note that frequent cancellations and last minute changes may result in being placed at the bottom of the waiting list for appointments or discharged as a patient from Restore.
